Your Ministry of Health Subsidy
The Breast Prosthesis Service Payment is a subsidy paid to people from the MOH who have undergone a partial or full mastectomy, either unilateral or bilateral, and/or have undergone a lumpectomy, have congenital needs, or have undergone reconstructive surgery, as specified by a specialist or general practitioner (GP).
At Denise’s Breast Care we keep our overheads to a minimum and this allows us to offer the highest quality products, at the prices others sell average products for.
If you are a NZ citizen, or ordinary resident in New Zealand you can claim for the entitlement of $613.33 per side for a four-year period (or $1226.66 GST incl for bilateral).
For your entitlement, Denise’s Breast Care will provide you with a prosthesis and 3 bras.
This service payment can be used for:
- Breast prosthesis or breast forms (these may include standard breast forms, swim form, foam filler, shell prosthesis and lumpectomy shells)
- Special bras to hold the breast prosthesis or breast forms
- Surgical bras
- Normal bras (the services include the modification of a normal bra to hold breast prostheses or breast forms, or modifications to alternative undergarments where a person cannot wear a bra due to their medical condition)
- Modified swimwear to hold a swim form
- Prosthetic nipples
As a supplier we will make this claim as stressless as possible and work with you during your fitting.
We can fill in details of the product and its cost on the Breast Prosthesis Claim Form, and then ask you to fill in your personal details and sign the form. We will then complete an additional form (stating that they are claiming on your behalf) and forward both forms to Sector Operations, with your medical certificate (where this is your first claim). You do not pay for the product yourself but keep a copy of the receipt we give you for your records.
